Vaccination Card Request

To ensure the health of everyone attending services, we are requiring those in attendance to provide us with proof of vaccination.  Please submit a photo (jpg or png) or pdf of your vaccination card and all that will be attending with you.  

If you cannot see the form, you need to sign into your B'nai Tikvah account.  If you haven't signed into your account before, you will need to set your password. At the top right, put in your email address, and then click "Forgot Password" and follow the instructions.  You will then be able to sign into your account.
